Therefore, if any faculty or staff members of our school still wish to participate in the on-campus (October 23rd afternoon) influenza vaccination, including those eligible for public funding and those purchasing with city funds, please contact the Health Center immediately before noon tomorrow, October 16th, Friday, to fill out the "Influenza Vaccination Consent and Assessment Form" (those aged 50 and above, please attach a copy of your ID card or health insurance card). This is for the final compilation and submission! Vaccination recipients: Full-time faculty and staff of our school, including long-term substitute teachers for over 3 months, club instructors, teaching support staff, temporary staff, and intern teachers, etc. On the day of on-campus vaccination (October 23rd afternoon), please bring your health insurance card.
