2. The aforementioned plan aims to establish a comprehensive care network for pregnant and postpartum mothers, strengthen and improve maternal and newborn care in our city, reduce the occurrence of maternal and infant accidents, and lower infant mortality rates. The information is as follows:
(1) Implementation Period: From January 1, 114 (2025) to December 31, 114 (2025).
(2) Target Population: General cases of pregnant or postpartum mothers registered in this city, or foreign pregnant or postpartum mothers whose spouses are registered in this city and who have not yet obtained an R.O.C. identification card.
(3) Service Content: Through face-to-face consultations and phone visits, providing "health education and consultation" and "care follow-up" from pregnancy to 6 months postpartum, or for special case needs. Home visit services will be provided based on individual needs, while also integrating relevant maternal and infant service resources in this city.
