| I. | This service plan provides various training courses or support services for visually impaired students in general schools (including kindergartens) below the university and college level. For service items, please refer to the attached "109 Visual Impairment (Student) Cross-Age Comprehensive Rehabilitation Service Plan". |
| II. | The service method of this service plan is to submit the "Application Form for Services Related to Visual Impairment" and "Application Form for Services Related to Preschool Children with Visual Impairment". Our association will then send personnel to discuss with the student's relevant teachers and parents, and provide services after assessment. |
| III. | This service plan provides telephone and online consultation for teachers, individuals, and parents of visually impaired individuals (students) at all stages of their lives, regarding issues related to daily living, education, training, and career for visually impaired students. |
| IV. | Related expenses: Provided by the "Cross-Age Comprehensive Rehabilitation Service Plan for the Visually Impaired" under our association. |
