| I. | In accordance with the letter from the Social Affairs Bureau of this Government, No. 1090055996 dated June 30, 2020. |
| II. | A total of 30 individuals are expected to be recognized in this case, as follows: |
| (1) | Model Persons with Disabilities: Registered in this city and holding a disability certificate, a total of 10 individuals will be recognized. |
| (2) | Model Family Caregivers for Persons with Disabilities: Registered in this city, personally providing long-term care for family members with disabilities who hold a disability certificate, a total of 10 individuals will be recognized. |
| (3) | Model Service Personnel for Persons with Disabilities: Individuals who have made significant contributions to promoting welfare for persons with disabilities and are actually providing services to persons with disabilities in this city, a total of 10 individuals will be recognized. |
| III. | Please submit the recommendation materials by Friday, July 31, 2020, via official letter to the Social Affairs Bureau of this Government. The postmark will be the basis for timeliness; late submissions will not be accepted. Please be sure to send the files via email to 103210@mail.tycg.gov.tw for successful registration. |
| IV. | The aforementioned plan can be downloaded directly from our bureau's website under "Welfare for Persons with Disabilities" / "International Day of Persons with Disabilities." |
