★ Important Notes:

1.Must bring your National Health Insurance card on the vaccination day!! You cannot be vaccinated without it!!

2.Please complete the online consent form before vaccination.

3.If you cannot be vaccinated at school on October 24th due to unforeseen circumstances,please have the student obtain a supplementary vaccination notice from the health center starting October 25th.Parents must bring the flu vaccine supplementary vaccination notice to take the student to a medical facility for flu vaccination.

★ How to Sign the Consent Form:

The campus flu vaccination uses an electronic system. We will provide a link and QR code to a signing webpage. Please have parents access NIAS to sign the consent form online. The online signingdeadline isbefore 12:00 on October 17, 113.，Before the deadline, parents can log in to the signing webpage through the above method to modify their vaccination preference.(If parents are unable to operate online, please have the student pick up a paper consent form from the health center. The paper consent form must be returned to the health center by October 10th.)
