113Influenza Vaccine Catch-up Vaccination Notice for Year 113

1. 10/24For students who were absent from school for the influenza vaccination, the Health Center uniformly issued catch-up vaccination notices to each class on 10/29. If parents did not receive the notice or if it was lost, students should go to the Health Center to print a replacement. If there was no intention to get the flu shot in the first place, parents do not need to respond to this notice. Thank you.

2.First ReissueThe date for the catch-up vaccination notice is:113Vaccination by November 30, Year 113. If the vaccination is scheduled after November 30, students should go to the Health Center to print a replacement notice.

3.Parents must bringthe student's influenza vaccine catch-up vaccination notice,the student's health insurance cardto the health center or contracted clinic for the influenza vaccination. If you do not bring the catch-up notice, you will not be able to get vaccinated!!
