Dear Parents:

The 1st dose of Human Papillomavirus (HPV) vaccine was completed at school on 9/19/114. The 2nd dose of HPV vaccine will be administered at school on 4/9/115.The Health Bureau has announced that on 4/9, students in the 2nd grade who have not received the 1st dose can also receive their 1st dose of HPV vaccine. Please note:

1. For those who have not received the 1st dose and will be vaccinated on 4/9::

Please have parents assist by checking "Agree" on the consent form,((Sign full name in block letters at the student and parent signature area), and return it to the health center by noon on 3/11 (Tuesday), so that the school can prepare the list and collect the consent forms. The vaccination will be administered on 4/9. Thank you!!

2. For those who will not receive the 1st dose of HPV vaccine:

Please have parents assist by checking "Disagree" and "Reason for Disagreement" on the consent form (Sign full name in block letters at the student and parent signature area), and return it to the health center by noon on 3/11 (Tuesday), so that the school can prepare the list and collect the consent forms. Thank you!!

3. For those who received the 1st dose of HPV vaccine at the health center, or transfer students who were vaccinated at their previous school:

The 2nd dose requires an interval of 6 months (180 days) from the 1st dose. Pleasereturn the pink HPV vaccination card to the health center by 3/11 (Tuesday). If the interval has reached 6 months, the 2nd dose can be administered on 4/9.If the pink HPV vaccination card is lost, you will need to get a replacement from the health center!

For those whose interval has reached 6 months, consent forms will be distributed for completion on 3/17 (Tuesday)!

Note: For students who received the 1st dose at school on 9/19/114, consent forms will be distributed to parents for completion on 3/17 (Tuesday)!
