Forwarding the information and application form for the "Food Box" program from the Chinese Andrew Charity Association, one copy each.

Students in need are requested to prepare the relevant documents and submit them to the Health Section of the Student Affairs Office for application by March 27, 115.

Required Documents for Application(Documents 1-5 must be provided; review and approval will only commence upon completion of all documents)

  1. Application Form (please download and print yourself)

  2. Copy of Household Registration Booklet or full Household Registration Transcript, one copy.

  3. Proof of low-income, low-middle-income, or impoverished status for the current year.

  4. One photo of the child (ID photo or casual photo is acceptable; electronic files can also be sent tomail@chaca.org.tw )

  5. If there are individuals with disabilities in the family, relevant supporting documents must be attached 

Subsidy Guidelines (Please read carefully before applying)

• If the application is approved, and food boxes are not collected monthly, the food box assistance will be suspended from the following month.

• If the application is approved, the assisted family must proactively go to the designated location specified by the association to collect the food box each month.

• If the application is approved, the association will schedule visits to the homes of assisted children for care and follow-up.

• If the application is approved, and the assisted family's contact information changes, they must proactively inform the association.

• Assisted children are obligated to participate in activities organized by the association from time to time. (e.g., writing thank-you letters, participating in the annual arts and crafts competition from June to September)

• Any materials provided by this association are strictly prohibited from being sold or exchanged for other valuable items by the assisted families. If there are other needs regarding the assisted materials, please inquire with the association. The association will review the situation and provide appropriate assistance. Violators will have their assistance terminated.
