1.Those who have completed 2 doses of HPV vaccine: have completed the public vaccination, please keep the vaccination card properly. After growing up (about 8-10 years later), decide whether to get the HPV vaccine at your own expense based on your needs.

2.Those who completed the first dose of HPV vaccine at school on 4/9:Please get the second dose 6 months later (after 115/10/9), **please be accompanied by a parent, bring the**HPVvaccination record card, andhealth insurance cardto the health center for vaccination.

3.If the second dose of HPV was not vaccinated at school on 4/9 (the first dose was vaccinated at school on 114/9/19):**Please go to the health center to collect the supplementary vaccination notice.****Please be accompanied by a parent,**bring the supplementary vaccination notice,HPVvaccination record card, andhealth insurance cardto the health center for vaccination.

4. If the first dose of HPV vaccination was at the health center:Please get the second dose at the health center after a full 6 months from the first dose, please be accompanied by a parent, bring theHPVvaccination record card, andhealth insurance cardto the health center for the second dose.

☆Students who need to get supplementary vaccinations at the health center, please note!! Because the HPV public vaccination program varies slightly each year, to protect your public vaccination rights, please complete it before 115/12/31!

☆If the HPV vaccination record card (pink) is lost, parents need to go to the health center to reapply for the record card! You need the record card to get the second dose!!
