Dear parents and graduating students:

1.The Health Center will return the health check-up record cards by115/6/3. Please keep them safe.

2.Graduating female students will receive the9-valent Human Papillomavirus (HPV) vaccination record card- Pink. Please keep it safe.

3.Female students who received the 9-valent Human Papillomavirus (HPV) vaccination record card before graduation have completed the 2 doses of the public-funded vaccination on campus in 8th grade!Please consider getting an additional HPV vaccination at your own expense after 10 years, when you are older, based on your needs!

4.Graduating female students will uniformly receive the "Taoyuan City Department of Health 115 HPV Vaccine Junior High School Female Student School Exit Vaccination Notice" from the Taoyuan City Department of Health.This notice is a reminder from the Department of Health to parents. If your daughter has never received the HPV vaccine or has not completed the 2 doses of the vaccine in 8th grade, and wishes to complete the vaccination, please follow the instructions on the notice and get vaccinated at the health center before 6/15!

5.Female students who have completed 2 doses of the HPV vaccine in 8th grade (before the age of 15):This means you have completed the public-funded HPV vaccination!You do not need to get a 3rd dose!Do not pay attention to the content of the notice!!
