September 9, 115, Seventh Grade New Student Health Examination Guidelines

Dear Parents, In accordance with the "Taoyuan City 115 Academic Year Elementary and Junior High School Student Health Examination Implementation Plan," we will be conducting health examinations for 7th-grade new students (Attachment 1). A medical team from **Chang Jung General Hospital** will visit the school (Examination Location: **School Gymnasium, 3rd Floor**) to conduct health examinations for students. 1. Examination Fee: **Free** 2. If you agree to the examination, please **do not be absent on the examination day**. If you are absent due to unavoidable reasons, please arrange for a make-up examination. 3. **Please wear sportswear on the examination day**. 4. For this examination, only the **chest and abdominal examination and follow-up electrocardiogram will require students to slightly lift their upper garments. [Male students] will need to lower their trousers to mid-thigh for the genitourinary examination. Appropriate privacy will be provided at the examination site, and teachers and school nurses will be present to assist and protect students' privacy. No other examinations will involve exposure of the body**. 5. Approximately **one month** after the examination, students will receive a "Health Examination Result Notice." **If any abnormalities are found in the examination results, please accompany your child to a medical institution for a follow-up examination with the "Health Examination Result Notice." The follow-up and corrective results should be noted on the reply slip and submitted to the homeroom teacher by 11/16.** The school will maintain health tracking records based on the follow-up results. 7. The consent form for new student health examinations should be **collected and submitted to the homeroom teacher by 8/21** (Attachment 2). If the form is not returned, it will be re-collected on the first day of school, 9/1. 8. If the consent form **checks the option to not have private parts examined at school**, it signifies that parents are willing to take their children to a hospital of their choice for a **self-funded examination**. A "Report for In-Person Hospital Examination" (Attachment 3) will be issued. Please submit the official report to the school's health center **before 9/30**. 9. **New Student Health Examination Day: September 9, 115 Academic Year (Wednesday)** 10. **Initial Urine Screening Date: September 9, 115 Academic Year (Wednesday)**. Urine samples will be collected on the day of the new student health examination. If the initial screening results are abnormal, parents will be notified of the urine re-examination date. If there are still abnormalities, parents will be notified of the blood test date. 11. The health examination precautions and calendar for Taoyuan Junior High School's 115 Academic Year 7th graders are as shown in (Attachment 4).
