| I. | In accordance with the letter of the Municipal Social Affairs Bureau, No. 1090055996, dated June 30, 2020. |
| II. | A total of 30 individuals are expected to be honored in this case, as detailed below: |
| (1) | Model Persons with Disabilities: Registered residents of this city with a disability certificate, a total of 10 individuals to be honored. |
| (2) | Model Family Caregivers for Persons with Disabilities: Registered residents of this city who personally provide long-term care for family members with disabilities, a total of 10 individuals to be honored. |
| (3) | Model Service Providers for Persons with Disabilities: Individuals who have made significant contributions to the welfare of persons with disabilities and are actively providing services to them in this city, a total of 10 individuals to be honored. |
| III. | Please submit the recommendation materials by Friday, July 31, 2020, via official letter to the Municipal Social Affairs Bureau. The postmark will be the basis for timeliness; late submissions will not be accepted. Please ensure that the files are also sent via email to 103210@mail.tycg.gov.tw for successful registration. |
| IV. | The aforementioned plan can be downloaded directly from our bureau's website under "Welfare for Persons with Disabilities" / "International Day of Persons with Disabilities." |
