For your information, Taoyuan General Hospital, Ministry of Health and Welfare, is offering a health check-up discount program for colleagues from various government agencies and schools under the Taoyuan City Government from 2020 to 2021. Colleagues are welcome to refer to and utilize this program.

Details:
1. In accordance with the letter from Taoyuan General Hospital, Ministry of Health and Welfare, dated October 28, 2020, ref: 1093907029.

2. To encourage colleagues to prioritize their self-health management and undergo regular health check-ups, the Taoyuan City Government has actively negotiated health check-up discount programs with medical institutions at or above the regional hospital level in our city, aiming to increase the rate of employee health check-ups and maintain the physical and mental well-being of our colleagues.

3. Taoyuan General Hospital, Ministry of Health and Welfare (hereinafter referred to as Taoyuan Hospital) has agreed to provide a discounted health check-up program. The relevant details are as follows:
(1) Health Check-up Programs: Four programs at NT$3,500, two programs at NT$7,000, and three programs at NT$16,000 are offered, totaling nine health check-up programs.
(2) Eligible Participants: Colleagues from various government agencies and schools under the Taoyuan City Government.
(3) Discount Period: From October 26, 2020, to December 31, 2021.
(4) Appointment and Application Services
1. Taoyuan Hospital agrees to reserve a certain number of slots every Monday to Friday for colleagues from various government agencies and schools under the Taoyuan City Government to book the aforementioned nine health check-up programs.
2. Online health check-up appointment services will be available starting from November 10, 2020 (excluding holidays). Those interested in scheduling an appointment should first review the precautions for each health check-up program and, according to the following appointment schedule, complete the health check-up appointment form and submit it to the personnel unit of their respective agency or school. The personnel will then make the online appointment on their behalf:
(1) Please complete the online appointment at least 2 weeks in advance. For example, if the health check-up is scheduled for November 20, 2020, please complete the online appointment by November 6 of the same year.
