In order to compile the list for the first phase of COVID-19 vaccination for our school's faculty and staff, please kindly cooperate with the following matters:

1. For faculty and staff who are eligible for the list and "do not wish" to receive the first dose of the novel coronavirus (COVID-19) vaccine, or who have "already completed the first dose" by now, please complete the attached "Waiver of Intent to Vaccinate" form and email it to tyjh0501@gm.tyjh.tyc.edu.tw by noon tomorrow (7/6). [You may also inform us by phone first at (03-3358282 ext. 710 or 720)]. If we do not receive the waiver by the deadline, you will be considered as willing to be listed for vaccination.

2. Regarding the "vaccination city" in the vaccination roster, our school's faculty and staff will be uniformly listed as "Taoyuan City." If you are eligible and wish to receive the first dose of the novel coronavirus (COVID-19) vaccine but intend to get vaccinated in "another city," please also inform us by phone by noon tomorrow (7/6). The HR office thanks everyone for their cooperation!

Note: The 대상 for this survey are as follows:
1. Full-time and part-time employees (including on-site personnel, outsourced personnel, etc.) who are "willing" to receive the first dose of the novel coronavirus (COVID-19) vaccine for the 110 academic year.
2. School volunteers and after-school (external) personnel are not included in this survey.
3. School personnel who are already announced vaccination targets by the Central Epidemic Command Center (e.g., medical staff, school nurses, nutritionists, pregnant women, dialysis patients, etc.) and have completed the first dose do not need to be listed.

*The scope of this roster compilation includes full-time and part-time faculty and staff, and on-campus workers (excluding school volunteers and after-school/external personnel). Those not included in this phase will be prioritized for the second phase.

*Faculty and staff employed in the 110 academic year are included in this reporting list. The Ministry of Education will notify about the second batch of targets later.
*Those who retire or resign after August 1, 110, will not be included in the roster.
*Those who transfer to a new school after August 1, 110, will be included in the first phase roster by the new school.
*Faculty and staff not listed in this roster (e.g., newly hired full-time teachers, substitute teachers) will be compiled in the next batch.
