| 1. | In accordance with the letter from the Social Affairs Bureau of our city government dated June 25, 110 (2021), document number 1100054332. |
| 2. | A total of 30 individuals are expected to be recognized in this program, categorized as follows: |
| (1) | Model Persons with Disabilities: 10 individuals who are registered residents of this city and hold a disability certificate. |
| (2) | Model Family Caregivers for Persons with Disabilities: 10 individuals who are registered residents of this city and personally provide long-term care for family members who hold a disability certificate. |
| (3) | Model Service Providers for Persons with Disabilities: 10 individuals who have made significant contributions to promoting welfare for persons with disabilities and are actively providing services to persons with disabilities in this city. |
| 3. | Please submit the recommendation materials to the Special Education Group of the Counseling Office by July 29 (Thursday), 110 (2021). |
| 4. | One copy of the aforementioned plan is attached. You can also download it from the homepage of our city's Social Affairs Bureau > Welfare Services > Services for Persons with Disabilities > International Day of Persons with Disabilities. |
