Please be informed that our university staff members who are interested in serving as invigilators for the 110th Special Examination for Local Government Civil Servants in the Taoyuan examination area are requested to obtain consent from their direct supervisor and submit the application form (attached) to the Human Resources Office by November 1st.
Details:
1. The examination dates and venues are as follows:
(1) Grade 3 Examination: December 11th to December 13th, 110 (3 days total). Venue: Chi-Ying Senior High School, Taoyuan City.
(2) Grade 4 Examination: December 11th to December 12th, 110 (2 days total). Venues: Chi-Ying Senior High School, Taoyuan City; Yongfeng Senior High School, Taoyuan City; Qingxi Junior High School.

2. In accordance with the severe infectious pneumonia (COVID-19) prevention measures, please complete the invigilator recommendation form as required. Relevant matters are as follows:
(1) When applying, you must select the examination venue and grade level you wish to participate in. Once selected by the Taoyuan City Government, you are required to participate in all examination duties throughout the entire period (according to the examination schedule).
(2) To facilitate the interface between the Examination Affairs and the Centers for Disease Control (CDC) of the Ministry of Health and Welfare (hereinafter referred to as the CDC) for comparison of individuals under home quarantine or home isolation who are not permitted to go out, please provide your national identification number when applying.
(3) If you have been vaccinated, please provide the vaccination date as recorded on your COVID-19 vaccination record card.
3. In response to COVID-19 prevention measures, all examination staff must be vaccinated. Therefore, priority will be given to those with invigilation experience and who have received at least one dose of the vaccine for this examination. If selected by the Taoyuan City Government and have not yet received the first dose, the Examination Affairs will create a special list for priority vaccination, to be submitted to the Central Epidemic Command Center (hereinafter referred to as the CECC) for approval. The following points are to be noted:
(1) The aforementioned list will only include examination staff who have not received the first dose of the vaccine. Therefore, please truthfully report your vaccination status on the attached form. False reporting will result in criminal liability. Furthermore, the type of vaccine is not yet determined and cannot be chosen (subject to the CECC's approval at the time), and you must agree to the following:
1. Upon agreeing to be vaccinated, you will perform all scheduled examination duties during the examination period (the examination period includes pre- and post-examination work, as well as rescheduled examination times due to typhoon contingency plans or postponements due to epidemic developments).
2. If you refuse to perform the scheduled examination duties without valid reason after receiving priority vaccination through the special program, your employing agency will take disciplinary action in accordance with relevant regulations (if unable to perform duties due to special circumstances, written consent from the Examination Affairs is required).
3. You have fully understood the potential adverse reactions after vaccination (for information on various vaccines, please refer to the CDC website/COVID-19 Prevention Zone/Vaccine Introduction).
(2) For examination staff who receive priority vaccination through the special program, the same position must be held by the same person throughout the entire examination period. No substitutions will be allowed during the period.
4. Personnel recommended by various agencies and schools must still be reviewed by the Taoyuan City Government in accordance with the
