2.Students who have been diagnosed with COVID-19cannot be vaccinated within3 months.Students who were diagnosed after

8should not submit a consent form.3.If students are under home isolation/ home quarantine/ enhanced self-health management/ self-health management/ self-epidemic prevention measures,they can be vaccinated after the relevant measures have ended and they have no suspected symptoms of COVID-19 infection.4.On the vaccination day, please bring yourVaccination Record Card(Yellow Card)and National Health Insurance card.
Both are required.5.A list must be created and the number of vaccinations reported in advance, so on-site vaccinations cannot be accepted.6.If the Vaccination Record Card(Yellow Card)is lost, please contact your local health center for a replacement.
