※Attention all 7th and 8th graders:
I. The vaccine consent form must be returned by noon on June 6th at the latest. (Any alterations on the consent form require a parent's full signature, otherwise, vaccination will not be permitted on the day.) As time is needed to organize data and create lists, late submissions will not be accepted!
II. The school has scheduled vaccinations for the afternoon of June 8th (Wednesday). Students who have submitted the consent form and agreed to be vaccinated must eat breakfast and lunch. Please arrive at school by 13:00 and go to your respective classrooms. First, perform a simple environmental disinfection. Please wear your school uniform on this day. Each class will listen to the broadcast to gather at the semi-outdoor basketball court. Every student must bring their health insurance card, vaccination card (yellow card), water bottle, and a ballpoint pen, and wait to be arranged for vaccination. School will be dismissed at 15:45 (the 8th period will not be held that day). If you need to leave school early for personal reasons, you must complete the leave application process before departing.
III. If a student is under home quarantine/home isolation/enhanced self-health management/self-health management/has any epidemic prevention related situation, they should proceed with vaccination after the relevant measures have ended and they show no symptoms of suspected COVID-19 infection.
IV. Confirmed COVID-19 cases cannot receive the COVID-19 vaccine within 3 months. The interval between the first and second doses must be 12 weeks, and the interval between the second and third doses must be 150 days.
V. 7th and 8th graders who choose not to be vaccinated should visit the school's homepage under the latest announcements at 13:00. There will be video learning on various topics. Students who do not get vaccinated do not need to come to school and should engage in self-learning at home.
