| 1. | In accordance with the letter from our government dated June 13, 111, with reference number 1110148326. |
| 2. | To reduce administrative burden, the issuing authority for the Designated Premises Isolation Notice for reissuance to confirmed cases of Severe Pneumonia with Novel Pathogens will be set as the local government health bureau, and it will be available from May 30 of this year (111). The details are as follows: |
| (1) | Personal information (name, contact number, address) and isolation address will be self-reported by the applicant. The start and end dates of isolation will be based on the testing date uploaded by the medical institution to the National Health Insurance Administration, Ministry of Health and Welfare. According to the policy for confirmed cases to be released from isolation, the system will set the logic for the isolation period as follows: |
| 1. | For confirmed cases with a judgment date from April 30 to May 7 of this year: The "Isolation Period" on the first page will be "Testing Date" plus 10 days; the "Start of Isolation Date" on the sixth page will be the "Testing Date," and the "Cancellation of Isolation Date" will be "Testing Date" plus 11 days. |
| 2. | For confirmed cases with a judgment date after May 8 of this year: The "Isolation Period" on the first page will be "Testing Date" plus 7 days; the "Start of Isolation Date" on the sixth page will be the "Testing Date," and the "Cancellation of Isolation Date" will be "Testing Date" plus 8 days. |
| (2) | If the information such as the start and end dates of isolation on the reissued Designated Premises Isolation Notice does not match the actual isolation situation of the individual, please contact the district health center directly for correction. |
