For your information, regarding the Taoyuan City Government's 111th year "Health Guardian" on-site health check event, all faculty and staff are welcome to participate enthusiastically.

Details:
I. To encourage colleagues to prioritize their self-health management, the Taoyuan City Government has partnered with the Far Eastern Memorial Hospital of the Far Eastern Yu-Chih Medical Foundation to organize an on-site health check event on July 28 (Thursday) and July 29 (Friday), 111th year, from 8:00 AM to 12:00 PM at the 4th-floor auditorium and parking lot of the Taoyuan District Office. On the day of the event, two discounted health check packages will be available: NT$4,500 and NT$7,000. These packages include items such as tumor marker screening and ultrasound examinations. The NT$7,000 package further includes special discounted tests such as the C13 urea breath test (valued at NT$1,500) and Vitamin D (valued at NT$1,000).

II. The event is open to all faculty and staff of the Taoyuan City Government and its affiliated agencies and schools (including civil servants, contract employees, technicians, workers, drivers, and survey assistants). Family members of employees and on-site walk-in registrations are not permitted. Official leave will be granted by the respective service agencies and schools upon verification. Furthermore, individuals who meet the eligibility criteria for health check subsidies as outlined in the "Subsidy Standards for Health Checks for Civil Servants, Contract Employees, and Workers of the Taoyuan City Government and its Affiliated Agencies and Schools" may apply for reimbursement from their service agency or school by presenting their payment receipts and adhering to the subsidy standards specified in the table.

III. In response to the COVID-19 pandemic and to prevent cluster infections, individuals with symptoms such as fever, loss of taste or smell, or respiratory issues on the day of the event are not permitted to participate. All participants are required to wear masks and have their temperature taken throughout the event. The Department of Personnel of the Taoyuan City Government will implement staggered check-in and health examination procedures based on the number of participants. Participants who have completed their examinations are requested to leave the venue promptly.

IV. Faculty and staff who wish to participate (please choose one date and package) are requested to complete the attached registration form (Excel file) and email it to the designated contact person in our school's Personnel Office (tyjh0501@gm.tyjh.tyc.edu.tw) by Wednesday, July 12, 111th year, and also notify the Personnel Office by phone. If the number of registrations for a particular date exceeds the limit (generally 150 people per day), participants will be required to adjust their participation date as coordinated by the organizing authority.
