| Subject: | Regarding the "5th Mathematics Activity Facilitator Training Workshop" organized by the Center for Mathematics and Science Education, National Taiwan Normal University, commissioned by the Ministry of Education's Department of Elementary and Secondary Education. | 1. | In accordance with the letter from the Center for Mathematics and Science Education, National Taiwan Normal University, dated November 9, 112 (Ref:師大數學中字第1121032474號). |
| 2. | Information regarding the subject matter is as follows: |
| (1) | Target Audience: Full-time teachers, substitute teachers, and contract teachers in junior high and elementary schools. |
| (2) | Workshop Capacity: Divided into three groups: lower elementary, upper elementary, and junior high. Each group can have up to three classes, with a principle of 28 participants per class, subject to adjustment based on registration. |
| (3) | Workshop Time and Location: Saturday, December 9, 112, from 9:00 AM to 4:20 PM, at Chuiyang Elementary School, Chiayi City (No. 605, Chuiyang Road, West District, Chiayi City 600). |
| (4) | Registration Method: Please complete the registration form by 12:00 PM on Wednesday, November 29, 112, at the following URL (https://forms.gle/AqNJv7geKuhi33Jt8). Late submissions will not be accepted. |
| (5) | Fees: This event is free of charge and includes lunch. Participants are responsible for their own insurance, accommodation, and transportation costs. |
| (6) | Certificate and Proof of Participation: |
| 1. | Mathematics teachers who meet the above qualifications and participate in the "Mathematics Activity Facilitator" training workshop organized by the Center for Mathematics and Science Education, and successfully complete all course content (must attend all modules for their respective group) to obtain certification, will be awarded the "5th Mathematics Activity Facilitator" certificate for that group. |
| 2. | To ensure training quality, participants who do not complete at least 2/3 of the module hours for the current session may audit subsequent workshops but will not receive the "5th Mathematics Activity Facilitator" certificate. |
| 3. | Only those who obtain the "5th Mathematics Activity Facilitator" qualification are eligible to apply for "Mathematics Foundation in the Classroom Teaching Practice." |
