Forwarding the notice regarding the Taoyuan City Government's 113th year "Health Gatekeeper" in-home health check event. All colleagues are welcome to participate.

Details:
1. To encourage colleagues to prioritize their health management, enhance the effectiveness of health check subsidies, and provide convenient health check services, the Taoyuan City Government has planned this health check event for the 113th year. After a public selection process, Min Sheng General Hospital has been appointed as the service provider. The details are as follows:
(1) Event Dates:
1. Session 1: July 18, 113 (Thursday), 8:00 AM to 12:00 PM.
2. Session 2: July 19, 113 (Friday), 8:00 AM to 12:00 PM.
(2) Event Location: 4th Floor Auditorium and Parking Lot (X-ray vehicle) of the Taoyuan District Office.
(3) Participants: Limited to colleagues of all agencies (schools) under the Taoyuan City Government. Family members are not allowed to participate. Agencies (schools) are requested to grant official leave as appropriate.
(4) Health Check Packages: This health check offers two discounted packages at NT$4,500 and NT$7,000, including common disease-related examinations such as tumor marker screening, ultrasound, and fundus photography.
(5) Other: If eligible for the Taoyuan City Government's health check subsidy, you may apply for reimbursement with the payment receipt from your agency (school).

2. Registration is by filling out a Google Form online. Please note the following:

(2) Registration Period: From now until June 21, 113 (Friday) at 2:00 PM, or until full.
(3) The Taoyuan City Government's Department of Personnel is scheduled to notify successful registrants and their respective agency (school) personnel units via email by July 1, 113 (Monday). If you do not receive a notification, please do not attend as on-site registration will not be available on the event day. If there are still available slots later, information regarding a second registration phase will be announced separately.
