| 1. | Due to frequent personnel changes among those handling volunteer service matters in various schools, to ensure a smoother issuance of volunteer service record books, please apply according to the "Application Form for Volunteer Service Record Books for Educational Purposes in this City" (as attached). You can also directly access the bureau's website (https://www.tyc.edu.tw/Default.aspx) under Convenient Services/File Download/Official File Download. After preparing all the necessary documents for the application of volunteer service record books, please mail or deliver them to this bureau for processing. |
