Vaccination Location:Taoyuan Junior High School - 3rd Floor Auditorium, Gymnasium

Vaccination Target:8thgrade girls (requires parental consent checked on the consent form)

Female students in the second year of junior high school in the 113 academic year who are citizens of the Republic of China and have never received the 9-valent HPV vaccine (i.e., female students who entered junior high in 112, born between September 2, 99 and September 1, 100).

Vaccination Date:113September 27 (Friday) from 08:20 to 09:30 AM, administered according to class assignment.

★On September 27, be sure to bringyour National Health Insurance cardto school.

★If unable to get vaccinated on that day due to unforeseen circumstances,please have the student obtain a supplementary vaccination notice from the health center, andparents must accompany the student to a public health center for the HPV vaccination. Please call the public health center in advance to inquire about their vaccination hours before going..
