| I. | In accordance with the letter Ref: T J G S X Z 1130135650 dated November 18, 113, from the Department of Secondary Education, Ministry of Education. |
| II. | To encourage elementary and junior high schools to actively promote oral health work and develop school-based oral health plans for other schools to adopt, promote, and observe, the aforementioned plan is hereby organized. |
| III. | How to participate in the aforementioned plan: Please fill out the recommended list (Attachment 1) in Word format (no seal required) and the scanned copy of the school self-assessment form (Attachment 2) with a seal by January 3, 114 (Friday), and send them via email to: 10032273@ms.tyc.edu.tw, so that our bureau can select and recommend the best candidates (the list will be notified separately). |
| IV. | Schools recommended by our bureau should upload the 113 academic year outstanding oral health school selection criteria form (Attachment 3) to the cloud space provided by the plan review team (URL to be provided separately) before 5:30 PM on February 14, 114 (Friday). |
