Subject: To inform you that the City Health Bureau, in order to protect the HPV vaccination rights of third-grade female students in our city, requests your school to cooperate with the arrangements as described in point 3. Please be informed.
Instructions:
1. In accordance with the letter from the City Health Bureau dated May 14, 114 (Year of the Republic of China), document number 1140041553.
2. The eligible recipients for free catch-up vaccination this year are third-grade female students in our city (those who entered junior high school in Year 111; age range from September 2, 98 to September 1, 99 of the Republic of China). If they have not received the HPV vaccine before, they must go to the city's district health centers before June 15, 114 to fill out the consent form for the first dose and complete the first dose of vaccination before June 30, 114. Subsidies will not be available after the deadline.
3. To protect the HPV vaccination rights of third-grade female students and increase vaccination rates, the City Health Bureau has printed "HPV Vaccine Notification for Junior High Third-Grade Girls Leaving School" to remind those who have not received any doses or have not completed all doses to get vaccinated as soon as possible to avoid losing their vaccination rights. Please have schools distribute these notices via the home-school communication folder. The notification slips will be delivered to each school soon. Please ensure that school staff are present to check and sign for them.

單」接種通知單，提醒未曾接種及尚未完成HPV疫苗所有劑
次者儘速接種疫苗，避免喪失接種權益，請學校運用家庭
聯絡本夾送發放，接種通知單近期將配送至各校，請各校
務必派員清點簽收。
