Forwarding the matter concerning the health check discount program for 2026-2027 provided by Chang Gung Memorial Hospital of Linkou Chang Gung Medical Foundation and 9 other medical institutions for the Taoyuan City Government. Colleagues are welcome to refer to and utilize this program.

Description:
I. To care for the health of colleagues and provide diverse choices, the Taoyuan City Government has actively engaged medical institutions within its jurisdiction to offer the aforementioned discount program, enabling colleagues to undergo examinations according to their needs, thereby enhancing health management effectiveness and workforce stability. Furthermore, to continue fostering a family-friendly workplace and improve the overall well-being of the organization, the scope of beneficiaries for the 2026-2027 period will continue to be extended to include employee dependents.

II. The aforementioned medical institutions and their offered health check discount programs are summarized as follows:
(I) Medical Institutions: Linkou Chang Gung Memorial Hospital of Chang Gung Medical Foundation, Taoyuan Chang Gung Memorial Hospital of Chang Gung Medical Foundation, Taoyuan Hospital of Ministry of Health and Welfare, Taoyuan General Hospital of the Armed Forces, Taoyuan Branch of Taipei Veterans General Hospital, Tian Sheng General Hospital of Tian Cheng Medical Foundation, Min Sheng General Hospital, St. Paul's Hospital of the Sisters of Charity of Sts. Anne and Joachim Medical Foundation, and Shin-Lan International Hospital.
(II) Eligible Recipients: Employees of all subordinate agencies (institutions), schools, and administrative legal persons of the Taoyuan City Government (including civil servants, contract employees, appointed personnel, janitors, technicians, drivers), resident police officers, sanitation workers, survey assistants, etc., as well as the dependents of the aforementioned personnel.
(III) Period of Validity: From January 1, 2026, to December 31, 2027.
(IV) Health Check Discount Programs: NT$3,500 to NT$32,000 (depending on the program offered by each medical institution).

III. Colleagues are welcome to arrange their health check-ups at an early date. If eligible recipients wish to utilize the aforementioned program, please proactively inform the medical institution when booking your health check-up and present your original employee ID on the day of the examination. For detailed information about the program, please contact the respective medical institutions directly.

IV. To facilitate colleagues' access to their health check-up data online at any time, please refer to the "Personnel Division Letter No. 1140249046 dated September 1, 2025" of the Taoyuan City Government (attached) to assist in informing them that they may sign a consent form, providing the medical institution where they underwent their health check-up, so that the examination data can be uploaded to the "National Health Insurance Health Bank."

V. Attached are one copy each of the list of discounted medical institutions for health check-ups for the Taoyuan City Government in 2026-2027 and the discount programs. Related information is also concurrently posted on the website of the Personnel Division of the Taoyuan City Government under "Business Information > Salary and Benefits > Welfare > Health Check-ups."
