| 二、 | 查本方案前經人事總處公開徵選，由富邦產物保險股份有限公司（以下簡稱富邦產險公司）承作，並配合相關規範自115年4月1日起調整個人保險專案組合。 |
| 三、 | 有關本方案調整後之DM及要保書相關資訊，人事總處業已公告於該總處全球資訊網（https://www.dgpa.gov.tw）最新消息、給與福利處「福利文康」區、公務福利e化平台及富邦產險公教員工旅遊平安卡網站（https://www.fubon.com/insurance/b2b2c/hwc/index.html）；或逕致電0809-019-888洽詢。 |
