| Subject: | Regarding the matters of "acceptance of claims during the semester transition period, implementation of student status review procedures, and revision of claim application forms" for the group insurance for students in the senior secondary and below education stages and early childhood groups (hereinafter referred to as group insurance), please be informed. |
| Explanation: |
| 1. | In accordance with the Ministry of Education, Department of Secondary and Early Childhood Education's letter No. 1156601965 dated July 29, 115. |
| 2. | Regarding the acceptance of claims during the semester transition period for academic years 114 and 115: |
| (1) | Claims for academic year 114 (and prior): Continue with the original procedures. Students continuing their studies are covered until July 31, 115. Graduates not continuing their studies are covered until August 31, 115. Claims should be submitted to the original insurance company (Cathay Life Insurance Co., Ltd., hereinafter referred to as Cathay Life). |
| (2) | Claims for academic year 115: As the new academic year's group insurance is still in the administrative process, claims for the new academic year will be processed after the subsequent enrollment briefing session is confirmed. Therefore, applications for claims for the new academic year 115 are temporarily not accepted. |
| 3. | To implement the statutory student status review procedures, please your school and early childhood service institutions handle the following matters: |
| (1) | According to Article 7 and Article 9 of the "Regulations on Group Insurance for Students in Senior Secondary and Below Education Stages and Early Childhood Service Institutions" and the "Regulations on the Effectiveness, Claims, Subsidies, and Management of Group Insurance for Students in Senior Secondary and Below Education Stages and Early Childhood Service Institutions" (hereinafter referred to as the Management Regulations), and Article 25 of the 114 academic year policy terms, schools and early childhood service institutions shall assist beneficiaries in applying for insurance benefits. Whether by affixing a seal on the claim application form or completing student status in the system, it is imperative to verify and confirm the "student's enrollment status," "legal guardian's identity and agency qualifications," and "accuracy of the application information." |
| (2) | After the school and early childhood service institutions complete the aforementioned confirmation procedures, the insurance company will review and process subsequent claims according to relevant regulations. |
| 4. | Cathay Life's group insurance claim application form will be revised in August 115. Starting from September 1, 115, all claims submitted for the period covered by Cathay Life insurance (including claims from academic year 114 and prior) must use the new application form (version 11508). Please your school and early childhood service institutions download the latest version from Cathay Life's official website (Claims Service Area). |
| 5. | If you have any questions, please contact Cathay Life's toll-free student insurance hotline: 0800-036-567. |
