According to the Taoyuan City2026academic year's National Elementary and Junior High School Student Health Examination Implementation Plan7thgrade new student health check(Attachment1), our school will have a health examination medical team composed ofChang-Jung General Hospitalvisit the school(Location:school gymnasium3rdfloor)to conduct health examinations for students:1.Examination Fee:Free

2.If you agree to the examination,please do not be absent on the day of the examination.

If you are absent due to illness, you should arrange for a make-up examination at another time.3.Please wear sportswear on the day of the examination.4.

For this examination, only the chest and abdomen will be examined. Students who require an electrocardiogram re-examination will need to slightly lift their tops. For male students, the genitourinary examination requires them to lower their pants to mid-thigh. Appropriate coverings will be provided at the examination site, and teachers and school nurses will be present to assist and protect students' privacy. Other examinations will not involve exposure of the body.5.After the examination is completed,

approximately one month later,a"Health Examination Result Notice"will be issued to the students.

If the examination results show any abnormalities, please bring the "Health Examination Result Notice" and accompany your child to a medical institution for a follow-up examination.On 11/16,please submit the re-examination and correction results noted on the reply slip to the homeroom teacher.The school will keep health tracking records based on the re-examination results.7.The consent form for new student health checks should be returned to the homeroom teacher by8/21(Attachment2), if not submitted, it will be reminded again on the first day of school, 9/1

.8.If the consent formchecks the option 'private parts will not be examined at school', it means that parents are willing to personally take their children to a hospital of their choiceto complete the examination at their own expense.A"Report of Self-Initiated Hospital Examination"(Attachment3)will be issued.

, urine will be tested on-site on the day of the new student health check. If the initial screening results are abnormal, a date for urine re-examination will be notified. 10.★初篩尿液檢查日期:115年9月9日(星期三)，新生健檢當日現場驗尿，若初篩結果異常，會再通知尿液複檢日期，若仍有異常，會通知家長及抽血檢查日期。
