| I. | This is in accordance with our organization's advocacy work for menstrual equity. |
| II. | The purpose of this survey is to gain a comprehensive understanding of the menstrual experiences of people in Taiwan, in order to promote gender-equitable and menstrual-friendly measures and policies. |
| III. | Survey participants and completion instructions: |
| (1) | Participants: Regardless of gender or age, anyone willing to share their menstrual experience. |
| (2) | Method: We kindly request your school to assist in printing the attached promotional material and posting it on school bulletin boards, or announcing it electronically on the school's official website, social media, or class groups. |
| (3) | This questionnaire is de-identified. The data will only be used for research analysis and will be kept confidential, without affecting the rights and interests of the participants. |
| IV. | Attached is a copy of the promotional material for this event. We kindly request your school to assist in announcing it on school bulletin boards, the official website, social media channels, or class groups. |
