According to the Taoyuan City2025Academic Year Elementary and Middle School Student Health Examination Implementation Plan,7thgrade new student health check(AttachmentOne),our school will have a health examination medical team fromChang-Jung General Hospitalvisit the campus(Location:Gymnasium:3rd Floor3樓),to conduct health examinations for students:

1.Examination Fee:Free

2.If you agree to the examination,please do not be absent on the day of the examination.If you are absent due to unavoidable reasons, please arrange for a make-up examination.

3.Please wear sportswear on the day of the examination.。

4.For this examination, onlychest and abdominal examinations will be performed. Students requiring an electrocardiogram re-examination will need to slightly lift their upper garment. For male students, the genitourinary examination requires lowering pants to mid-thigh. Appropriate covering will be provided during these examinations, and teachers and school nurses will be present to assist and protect student privacy. No other body parts will be exposed during the examination.。

5.After the examination,approximately one month later,students will receive a"Health Examination Result Notice".。If the examination results show any abnormalities, please bring the "Health Examination Result Notice" and accompany your child to a medical facility for a follow-up examination. On 11/24, please have the follow-up and correction results noted on the reply slip and submit it to the homeroom teacher.11/24將複查及矯治結果註記於回條上交給導師。The school will maintain health tracking records based on the follow-up examination results.

7.The reply slip for the new student health check consent form is due on 8/22. Please return it to the homeroom teacher.同意書回條於8/22繳交回給導師收齊(AttachmentTwo),If the consent form is not returned, it will be reminded again on the first day of school, 9/1.9/1開學日會    再催繳。

8.If the consent form indicates "private parts not to be examined at school",勾選私密部位不在校檢查it means parents are willing to take their children to a hospital of their choiceto complete the examination at their own expense.

  成檢查，A "Self-Attended Hospital Examination Report"「自行到院檢查報告書」(AttachmentThree),will be issued.Please submit the official report to the school's Health Center before 9/30.9/30前交正式報告書至學校   健康中心

9.New Student Health Check Date:2025Academic YearSeptember月12th(Friday星期五).
